Access to information request

This useful form is available to send to the City a request for access to a document or a personal information.

The information given under Section ‘Identification of the requestor’ is kept confidential.

If you need help in identifying the document you are looking for, or if you need information concerning An act respecting Access to documents held by public bodies and the Protection of personal information, contact the City Clerk who is the person in charge of access for the City.

A request must be made in writing and must be sufficiently detailed to allow the person in charge to answer you. Thus, you can mention the title of the document, the name of the author or the subject.

Upon receipt of this form, the person in charge will acknowledge receipt of your request and will answer within 20 days. An additionnal 10 days may be requested.

The access to document request is free. However, the City charges a fee not exceeding the cost of transcribing, transmitting or copying the document. The person in charge will give you the required information before proceeding.

Affidavit (general)

An affidavit is required when a person must swear under oath or make a solemn declaration.

The person writes a declaration or completes one of the following forms. The document MUST be signed before a commissioner of oaths.

It is important to mention that the commissioner of oaths does not approve the content of the document but only the authenticity of the signature.

Annual seniors grant

Dollard-des-Ormeaux seniors may be eligible for an annual grant of $150 for tenants or $300 for homeowners, based on the valuation of their residence and their annual income.

Authorization to travel with young children

Please read the text below to prepare your documents correctly.

This document is often required by customs when a child is travelling with only one parent or with a third party.

The parent authorizing the child to travel with only one parent or a third party must go to a commissioner of oaths with a letter giving the following:

  • - the name of the person who will be travelling with the child and his/her relationship to the child;
  • - the travel destination including the city and country;
  • - the date and time of departure and arrival.

The parent MUST sign the letter before the commissioner of oaths and provide the following identification:

  • Two signed pieces of identification, at least one of which has a photo (Medicare card, driver's licence, passport, citizenship card, permanent resident card or social insurance card). You may also provide a tax bill, lease or a public utility bill.

Birth certificates are no longer available at the City Hall but you may obtain a request form at these locations:

Claims

Anyone who feels that they have suffered material damages which they intend to claim against the City must, within FIFTEEN (15) days of the incident, send a written notice to the City.

Similarly, it is strongly recommended that individuals who, as a result of an accident suffered bodily injuries for which they plan to claim damages and interest from the City, must submit a written notice as quickly as possible.

The written notice can be sent at city@ddo.qc.ca, mailed or faxed to 514-684-0125.

The notice must be written by the claimant and must include the following information:

  • last and first name of the claimant;
  • address of the claimant;
  • cause of the damages;
  • date and location of the incident.

The claimant may use the attached form or write a letter.

Form - Claims

Dog and cat licenses

All dogs must be registered yearly for the period of August 1 to July 31. Cats have to be registered every three years. To register your animal, please complete the following form and return it with your cheque. An identification tag will be mailed to you. You may fill in the form and pay the annual registration fee at the cashier's desk at City Hall, 12001 De Salaberry Boulevard.

The annual fee for a dog licence:
Spayed: $15
Not spayed: $25

Invitation letter or certificate of responsibility

This document is required to obtain a tourism visa to stay in Canada. The attached document must be filled out by the person who is acting as guarantor for the visitor. This form MUST be signed at our offices before one of our Commissioners of oaths.

Life Certificate

A life certificate is required for persons who receive an income from a foreign country since the organization that deposits the income insists on proof that the recipient is still alive. The recipient generally receives a form but the attached form can also be used. The form MUST be sign at our offices before one of our commissioners of oaths.

If you want to prune or cut down a tree located in the front, side or back yard of your property, you must fill out the following form and return it to City Hall located at 12,001 De Salaberry Blvd. or at Public Works located at 100 Glenmore. You can also return it by mail at the following address:

Your request will then be forwarded to a City Inspector for proper follow-up and approval.

Residency certificate

This document confirms the address of a person. The person who wishes to obtain this certificate must bring a letter or the attached form to our commissioner of oaths. The applicant must show two proofs of residence such as a tax bill, lease, public utility bill (except a telephone bill) or driver's licence and two signed pieces of identification, at least one of which has a photo (Medicare card, driver's licence, passport, citizenship card, permanent resident card or social insurance card).
