Proof that you were 65 or over on January 1, 2022 (health insurance card, driver’s licence, birth certificate).
A copy of your 2021 T4A (OAS), Statement of Old Age Security, including confirmation of receipt of the Guaranteed Income Supplement, issued by Employment and Social Development Canada.
A copy of your Municipal tax account.
