N.B.: Please include with your request:
Proof that you were 65 or over on January 1, 2025 (health insurance card, driver’s licence, birth certificate,2024 T4A (OAS) statement, etc.).
A confirmation of receipt of the Guaranteed Income Supplement, issued by Service Canada in July 2024, covering the period of July 2024 to June 2025.
If you are an owner, a copy of your Municipal tax bill. Your property assessment cannot exceed $759,200.
Information
Note : The amount is to be divided equally amongst eligible applicants living at the same address if the form is submitted jointly. If the full amount has already been issued to a household in 2025, no additional amounts will be paid out. The Support for Seniors program is not retroactive.
